First+Plus has a List of Covered Drugs (Formulary). This list tells you which Part D prescription drugs are covered by our plan.  The drugs on our formulary are only those covered under Medicare Part D. We will generally cover a drug on the plan’s formulary as long as other coverage rules are followed and the drug is medically necessary, meaning reasonable and necessary for treatment of your illness or injury.  Medicare has approved our formulary.

Our drug formulary includes both brand-name and generic drugs. A generic drug is a prescription drug that has the same active ingredients as the brand-name drug. It works just as well as the brand-name drug, but it costs less. There are generic drug substitutes available for many brand-name drugs.

Although changes to the formulary may occur throughout the year. Generally, if you are taking a drug on our 2010 formulary that was covered at the beginning of the year, we will not discontinue or reduce coverage of the drug during the 2010 coverage year except when a new, less expensive generic drug becomes available or when new adverse information about the safety or effectiveness of a drug is released. However, Other types of formulary changes, such as removing a drug from our formulary, will not affect members who are currently taking the drug.

If a drug is removed from our formulary, or prior authorization, quantity limits and/or step therapy restrictions are added we must notify affected members of the change at least 60 days before the change becomes effective, or at the time the member requests a refill of the drug, at which time the member will receive a 60-day supply of the drug. If changes occur throughout the year, these will be mailed in a separate notification to all members. In addition, any updates will be posted on this web site throughout the year 2010.
